According to the IDP return fact sheet as of 31st December 2017, total 16,998 registered families have been returned to Tehsil Ladha, Makeen and Shawal in 2017, while 4,926 families are still in displacement. Along with the registered caseload, un-registered IDPs families have also been returned to these tehsils.
In current scenario, most of the families have been returned to Tehsil Ladha and Makeen, while on the other hand, due to damaged infrastructure, and non-functional health facilities, the population facing sever concerns regarding the healthcare services, which is negatively impacting the sustainable return process. According to EHSAR’s Assessment Report, the services delivery in health sector is lacking due to the insufficient resources with the government. The report also indicates that there are total 27 government health facilities in Tehsil Ladha and Makeen, but most of them are not functional, either due to damaged infrastructure or lacking in human resources. According to the preliminary findings of Vulnerability Assessment, 67% of the population are facing sever or moderate difficulties in accessing health facilities, while 62% of the population were not satisfied with the available health services. Furthermore, the VA report indicate that 37% of the PLWs never received medical help during their last pregnancy, while 51% of the deliveries have been done at homes. Overall, 88% of PLWs haven’t been visited by LHWs in last three months. The report further indicates that 64% of children didn’t vaccinated through EPI program.
Keeping in view the heavy caseload, and the gaps identified, the need for the proposed intervention is much higher. The proposed project is designed to fill the gaps in the delivery of essential primary health care services including reproductive health and mother and child health care services to the recently returned population of South Waziristan Agency, by targeting 4 government health facilities. Through the proposed project 65,500 individuals will be assisted in 09 months with the following activities.
Revitalization of health system in target areas through provision of Pre-fabricated health facilities (two) and then services delivery
Minor Repair works in two health facilities and installation of small-scale solar system in targeted health facilities as alternate electric power generators.
Provision of medical equipment and necessary furniture to the targeted health facilities to ensure the smooth operations of PHC services
Ensure complete package of primary health care services in the targeted health facilities and through mobile camps in the far-flung villages, and establish disease surveillance and response system.
Provision of life saving mother and child health and reproductive health services through filling gaps in the service delivery to women and children.
Full time BeMONC and referral services including ambulance services for complicated cases to nearest AHQ/DHQ Hospitals.
Provision of mental health and psychosocial support, especially to female, having mental health issues, through female Psychologist.
Provision of essential lifesaving medicine and supplements for PLWs in all the targeted health facilities for the entire caseload
Support the government in routine immunization to children, vitamin A supplementation and TT vaccination to women, by identifying the defaulter, and refer them to the government EPI Technician for further action
Disease surveillance and response to mitigate the risks of outbreaks of communicable diseases such as URTI, cholera and measles, in the targeted areas.
Linkages with nutrition and WASH, to mitigate the risks of outbreaks of waterborne diseases and acute malnutrition in the areas.
Provision of health and hygiene sessions to community including Girls, boys, men and women.</narrative></description><participating-org ref="" role="2" type="22" activity-id=""><narrative>Education revitalize the health system in return areas of Central Kurram Agency to deliver life saving health services to men, women, children to reduce morbidity and mortality.</narrative></title><description type="2"><narrative>After the military operation in FATA agencies, the social service infrastructure including the health facilities have been destroyed especially in korram agency. The TDP returned families are deprived of quality PHC/MNCH services because of non-availability of infrastructure, lacking of health staff, medicines and other health supplies. The health information fabric is destroyed and potential diseases outbreak information is not existing in Korram Agency. Under the proposed project, SHED will fill the gaps in health services through revitalize and operationalize the health care system through mobile and static health care services, provision of pre-fabricated structure, necessary equipment, medicines for PHC ,psycho-social support and MNCH services in the following areas of central Kurram (suggested /endorsed by Agency Surgeon and Securities Agencies-Endorsement letter attached) of Kurram Agency:
1. Mian Kali, Masozai, Central Kurram
2. Nika Ziarat, Masozai, Central Kurram
3. Laky Kali, Masozai, Central Kurram
4. Hindu Kali on Dogar Road, Central Kurram

According to Agency Surgeon and security agencies, these areas have no health facilities. People of the area have to travel to Sadda for PHC services taking 3 to 4 hours to get there. The FATA health department/Pak Army authorities are committed to take them over in future to provide staff and other facilities)

SHED will install solarized pre-fabricated structure in fully damaged health facilities in order to make them operational. SHED will also form four health teams for holding free static amp mobile medical camps in the far away area of the catchment population of the areas. Each team will consist of 1 MO, 1 LHV, 1 MT, 2 SMs and 1 female Psychologist. Each Medical officer will be responsible to lead 1 health team. Total 704 Static ampMobile Medical Camps in all the 4 selected areas in 8 months period will be conducted and will reach to 32480 beneficiaries. The teams will conduct OPDs to examine 21120 patients under PHC, 7040 MNCH patients and will provide psycho-social counseling to 3520. The team will also provide MNCH services to the mothers/PLWs about ANC, PNC, safe delivery growth monitoring and will distribute 400 safe delivery kits and 400 new born baby kits on need basis. The reproductive health services will include family planning counseling, spacing between births and use of contraceptives etc. Referral mechanism will be established and strengthened with the nearby tertiary hospitals for complicated cases through ambulance service.

SHED will advocate for provision of vaccinator with Agency Health Office where the vaccinators are not available to ensure that children and PLWs are properly vaccinated in the target health facilities.

SHED social mobilization team will introduce the project to the local communities and form Health Advisory Committees (HACs), 2 for each health facility (male amp female) with membership from 8 to 10 in the catchment area of the selected health facilities. The HAC members will be trained on their role.

SHED will conduct training to 20 Govt. health staff of the of the Agency health office in a day-long session on record keeping, medicine rationalization, Disease Early Warning System (DEWS) for early detection, notifying, response mechanism for outbreak with reporting and MNCH services.

The Psychologist will provide counseling to patients suffering from stress situation in SMCs/MMCs. The acute stress cases will be referred to the nearest trauma centers for further treatment.

For sustainability, the pre-fabricated structure, solar system, medical equipment and furniture will be provided and will be handed over to the government line department after project completion to strengthen and support for provision of quality health services to the affected population. The staff of the health facilities will be trained on record keeping, medicine rationalization, patients’ management and DEWS reporting.

Based on the Vulnerability assessment report, Shelter cash assistance to 320 families of Ali sher zai, Zaimusht and Khoidad khel of Kurram Agency will be based on their vulnerability.
In March 2015, the Pakistan army declared the area clear from the militants and the TDPs started return to their hometown to face the harsh reality of their destroyed houses and infrastructure. The public infrastructure of schools, health units, roads and bridges etc. were destroyed during the military operation and continuous fighting. UNHCR was the first humanitarian organization that stepped to provide emergency assistance to the families (tents and NFIs) to cater their immediate shelter needs. FRD was part of the interventions as partner in Kurram agency to facilitate the families. Although the interventions provided immediate relief, but there is a need for permanent solutions to the shelter needs. There had already been some interventions in the agency under the PHPF but the need are high and there had been new returns since the last interventions, therefore is a need to provide assistance to the families of Ali sher zai, Zaimusht and Khoidad khel tribes. The area have seen severe devastation and are relatively new in term of returns. The families need permanent shelter which can help them escape the harsh weather conditions of the area and provide them security and protection with women and children and the elderly who are extremely vulnerable in such conditions. The project will complement the govt. efforts to resettle the families’ at their areas of origin through providing a long term permanent solution to their shelter needs.
The project will target 320 of the most vulnerable families of kuram Agency at their areas of return with the provision of cash assistance for rehabilitation of their houses in accordance with their indigenous construction practices, with incorporation of the possible DRR measures and Guidance by the FRD technical staff.FRD has already been part of shelter assistance at Kurram agency and completed a project for the assistance of 310 families under the PHPF in 2016. FRD is still is on ground as part of the WFP interventions in the area. The proposed interventions will be the part of phase two interventions as per the shelter cluster strategy to enable the families to have suitable shelter solutions to their needs and in line with the PHPF allocation strategy for shelter sector. address the immediate critical WASH needs of returned population of Tehsils Ladha and Makeen, South Waziristan Agency (SWA) in FATA.</narrative></title><description type="2"><narrative>According to PHPF allocation strategy 2018, FATA remains one of the least developed regions in Pakistan having experienced man-made and natural disasters for many years. From 2008 to 2014 more than 5 million people were displaced from the FATA due to the security operations by the military against non-state armed groups. Government initiated the return process with assistance of humanitarian partners, and have facilitated 438,657 (93%) of the displaced families to return to FATA including 74,590 families to South Waziristan Agency (SWA), There remains a total of 29,059 families who are yet to return.
Despite the efforts of government and humanitarian partners in providing safe drinking water, half of the people are using unprotected water sources (i.e. canals, rivers, open wells, ponds, springs, etc.). Poor sanitation, lack of water, low water quality, and inappropriate hygiene practices are detrimental to the health of school-aged children. The physical environment and cleanliness of school facilities significantly affects the health, attendance rates, and well-being of children. Disease spreads rapidly where hand-washing facilities and soap are not available, and where toilets are in poor condition. A lack or limited supply of electricity limits the operation of existing water supply schemes, while the rocky terrain makes hand pumps generally ineffective. The capacity of FATA line departments is also insufficient due to limited technical staff and presence at the agency level, making it harder to ensure sustainability of current initiatives. WASH cluster KP / FATA and, IVAP detailed needs assessment analysis of SWA shows 55,728 people in South Waziristan require immediate assistance in restoration of drinking water supply 37,155 people require assistance in terms of sanitation and 84,432 people require awareness on suitable hygiene practices.
CARE assessment findings confirms WASH facilities and services are the critical and immediate needs of return populations in SWA. Currently, the communities are heavily relying on unsafe drinking water sources and available water is insufficient and inadequate in-terms quality and quantity. Absence of sanitation facilities and services have led to open defecation practices and poor hygiene conditions resulting in increased incidence of waterborne diseases.
In order to meet the identified critical immediate and urgent needs, CARE will focus on interventions outlined below,
1- Rehabilitation of 07 existing damaged/affected water supply systems named Zawar Killi, Shaga Bromi Khel,Shabi Khel, Mishta, Landi Khel Shaktoi, Zodai Shaktoi in Tehsil Ladha and Shaktoi Chalarai and Zair sar in Tehsil Makeen. Provision of drinking water to school children and patients through 04 drinking water supply schemes in institution (schools and health facilities).
2- Installation of 15 communal hand pumps to improve access to quality and sufficient quantity of drinking water.
4- Provision of water 04 facilities Rehabilitation of existing water supply systems, rehabilitation of wells and installation of 08 hand pumps) in the institutions
5- Provision of sanitation facilities (73 latrines and hand washing places) in the institutions
6- Hygiene promotion campaign and session in the schools
7- Water quality testing by using DelAgua water quality testing kit and Pakistan Council of Research in Water Resources - PCRWR laboratory and development of water safety plans in line with social gender inclusive approach.
8- Implementation of PHAST, community mobilization that is culturally appropriate, secure, user friendly and gender appropriate. (Provision of 1000 sanitation kit and cash for work @PKR 5,000 for latrines construction).
9- Capacity Building of Government and civil society on WASH related training through WASH cluster KP / FATA.
10- Gender inclusive hygiene promotion campaigns and sessions, IEC material dissemination and distribution of 1000 family hygiene kits.</narrative></description><participating-org ref="" role="2" In each constructed latrine in schools and health facilities a separate cleaning kit will be provided. A total 20 latrine cleaning kits (4x3=12 schools +4x2=8 health facilities) will be provided.
In addition to provision of clean drinking water and sanitation facilities in schools and health facilities, the project is aiming to organize 5 trainings for teacher and health facilities staff on hygiene promotion. 75 individuals (25 health staff+ 50 teachers) will be trained on improved hygiene practices. To improve the personal hygiene practices of school children the project plans to target 1,440 school children both girls and boys through school specific hygiene campaigns in 4 girls and boys’ schools by using CHAST approach. Sessions will focus on children representing a cluster of households, different social background, economic groups and age.
Also PRDS plans to provide humanitarian assistant on war steps basis to the return family of South Waziristan Agency through rehabilitation of 9 communal water schemes. In addition to the convenient access of return families to safe and clean drinking water sources, 1000 return families will be provided sanitation kits for basic latrines along with cash for work support to make sure the returnees with construction of their latrines. The provision of sanitation kit will minimize the health risks associated with open defecation and will enable returnees to construct their own household latrine on a self-help basis that is sustainable and contributes to longer-term development goals.
Clean water alone will not prevent water and sanitation related diseases. In the same target zones as water and sanitation interventions 14000 beneficiaries will be reached through mass awareness on hygiene education using PHAST techniques. Apart of awareness sessions/campaign on hygiene education, 1000 family hygiene kits will be distributed to ensure the return families with improved hygiene at an individual and household level.)
The project has been introduced with the concerned departments and they endorsed PRDS with a list of Schools and Health facilities for the delivery of proposed WASH service. (Letters of endorsement by the departments along with lists of schools and health facilities are uploaded in document tab of GMS
Likewise the recommended list of schools and health facilities by the agency concerned departments, the communal schemes which are being proposed for the rehabilitation have been prioritized by the political administration of South Waziristan Agency. endorsement letter is uploaded in the document tab of GMS)

BANNU</narrative></title><description type="2"><narrative>As a result of the military operations started in June 2014 in North Waziristan Agency (NWA), there was a massive displacement from NWA to the settled Districts of Khyber Pakhtunkhwa Province. District Bannu being the neighboring District received the major influx of TDPs. FATA Disaster Management Authority (FDMA) with the support of Pakistan Army, established a Camp to accommodate TDPs from NWA. The Camp has the capacity of accommodating 5000 families. In order to provide all the essential services (Shelter, Health, Securit) FDMA with the support of Pakistan Army mobilized all the available resources.

PRCS being auxiliary to the Government in Disaster Preparedness and Disaster Response, extended its services by providing Health Care Services throughout the Camp.

During disaster and emergency women and children and elder people are highly vulnerable to diseases and basic life essentials. Women and children are badly affected due to poor hygiene conditions, and in NWA case displaced population underwent different security checks and long pedestrian travelling through rough and hilly areas. Pregnant women and elders travelling with chronic diseases are at high risk due to non availability of medicines and medical consultation during illness. The displaced population of all ages badly effected by the water borne diseases due to consumption of contaminated water.

PRCS, with the financial and technical support of ICRC, established a field Health Unit which has the capacity to provide following services:
General OPD services along with provision of free medicines
	Emergency Response Services
	Referral Services
	Health amp Hygiene Promotion
	Basic Diagnostic services

PRCS has been successfully running its Field Health Unit and has been providing quality health care services to more than 175,000 patients since March 2015.
